Master the surgical treatment of acromioclavicular dislocation. This training offers a technical immersion in osteosynthesis with the TightRope device, emphasizing precision in demarcation, dissection, and creation of bone tunnels, aiming for a favorable functional and aesthetic outcome.
Training Focus:
- Surgical treatment of acromioclavicular dislocation.
- Utilization of the TightRope device.
- Technique for creating bone tunnels in the clavicle and coracoid process.
- Reduction and stabilization of the acromioclavicular joint.
Detailed Content:
-
Precise Radiographic Evaluation: Methodology for identifying the base of the coracoid process using anteroposterior (AP) and apical (axillary) images for device insertion planning.
- Optimized Demarcation and Surgical Access: Planning of the longitudinal incision from the coracoid process to the lateral third of the clavicle, with the use of a diluted adrenaline solution to minimize subcutaneous bleeding and ensure a clear field.
- Controlled Dissection and Tissue Preservation: Execution of single longitudinal incisions, maintaining the same direction to avoid soft tissue injuries and bleeding, with slow progression for optimized results.
- Exposure and Palpation of Key Structures: Technique for palpating the anterior end of the clavicle and the coracoid process with digital exploration and adequate retraction for safe exposure.
- Creation of Strategic Bone Tunnels: Positioning of the clavicular tunnel in the anterior middle third, close to the base of the coracoid, and the coracoid tunnel as close as possible to its base, to prevent anterior deviation of the clavicle and postoperative failure.
- Insertion and Adjustment of the TightRope Device: Sequential procedure of passing the TightRope through the clavicle and coracoid holes, with alternate traction of the sutures for reduction and fixation of the dislocation.
- Layered Closure and Superior Aesthetic Result: Surgical closure performed in layers, with intradermal suture for an imperceptible aesthetic result.
Included Material:
- Detailed PDF: This PDF offers a practical summary of the procedure, addressing the condition of acromioclavicular dislocation, radiographic evaluation, steps for surgical access and preparation, dissection technique, creation and positioning of bone tunnels in the clavicle and coracoid, insertion and adjustment of the TightRope device for dislocation reduction, layered closure, and expected outcomes.
