Detailed Content:
- Provisional Fixation and Assessment: The training begins with a manual reduction maneuver and provisional stabilization with a 2.5 mm Kirschner wire, aiming for good initial fracture positioning.
- Volar Surgical Approach: The approach is made with an incision over the flexor carpi radialis. It addresses the sectioning of the fascia covering this tendon and the careful progression of dissection, plane by plane, to avoid muscle cuts and bleeding. The radial artery is located, and the flexor pollicis longus and pronator quadratus are identified. A needle is used to identify the exact point of the joint, delimiting the distal limit of the approach. It concludes with an L-shaped incision in the pronator quadratus.
- Radius Exposure and Plate Implantation: The radius is exposed, and a periosteal elevator is used to prepare the plate bed. With the fracture already reduced by provisional fixation, screw placement must respect the radial inclination to prevent intra-articular positioning. It is recommended to maintain reduction manually or use additional wires to avoid loss of reduction.
- Closure and Results: The access through the pronator quadratus is sutured to cover the plate. The final result is satisfactory, with well-positioned screws, maintained radial inclination, and a well-fixed fracture. Closure is performed layer by layer up to the skin.
