Master the surgical treatment of dorsal Barton's fracture of the radius, which presents with articular displacement and requires plate stabilization via a dorsal approach. This training offers a practical immersion in the reduction technique, surgical access, implant selection, and fixation, aiming to restore wrist anatomy and function, allowing for early postoperative mobility, especially in elderly patients.
Training Focus:
- Surgical treatment of dorsal Barton's fractures of the radius.
- Indirect reduction technique.
- Surgical access and implant selection.
- Fixation to restore wrist anatomy and function.
- Early postoperative mobility.
Detailed Content:
- Assessment and Surgical Access: The training begins with radiographic assessment for reduction and implant planning. It covers marking the access over Lister's tubercle, identification of muscle compartments, and tendon retraction to expose the fracture.
- Implant Selection and Reduction: Exploration of the styloid region to identify comminution is presented. The course recommends delicate implants to prevent tendon irritation. The indirect reduction technique is demonstrated, using a proximal screw to reduce the fragment through the plate, with traction maneuvers if necessary.
- Fixation and Checks: After the main plate, the training teaches how to use a second low-profile plate for detached dorsal fragments, aiming to restore the articular region. Checks are taught to ensure no tendons are under the plate and that the screw holes do not violate the joint.
- Closure and Results: The plates are positioned flush with the bone, ensuring tendons are free. The training covers assessment of the reduction in terms of radial length, angulation, and inclination, and immediate mobilization in elderly patients postoperatively. Skin closure concludes the procedure.
Included Material:
- Detailed PDF: Covers initial assessment, surgical access, exploration and implant selection, indirect reduction, fixation of additional fragments, checks, final assessment, and closure. Also includes a practical summary of the procedure with key points.
