• Surgical Approach and Exposure: Lateral approach with Kaplan interval, dermographic markings on the lateral epicondyle. Planar access with hemostasis. Multifragmentary fracture requiring prosthesis. Annular ligament opening and osteotomy with radial neck preservation.
• Medullary Canal Preparation: Cushion under the elbow for slight varus. Manual reaming with intimate fit to the medullary canal for good seating of the uncemented implant.
• Osteotomy Testing and Regularization: Test implant diameter with manual rotation of the arm/hand. Preference for a smaller diameter if in doubt. Regularization of the osteotomy and use of a trial component according to radial head measurement.
• Final Implant: Implant must align with the articular surface of the olecranon. Adjustments to the osteotomy, compaction of the component with a hammer. Movement test without impingement after reduction.
